The New York State Division of Homeland Security and Emergency Services in collaboration with affected counties has developed an online tool to report damage from the most recent storms. The purpose of the Damage Self Report tool is to help State and County officials collect data that can be used to identify all damage that may require additional requests for federal assistance. This is not an application for relief programs.
All damage reporting is voluntary and is not a substitute for reporting damage to your insurance agency nor is it a guarantee of disaster relief.
Please submit one survey for each affected housing unit or business. This form is only applicable to properties damaged in New York State.
The information you need to have available includes:
- Date the damage occurred
- Physical address of the damaged property
- Whether or not it is a primary residence
- Whether you rent or own the property
- Whether the damage is to a residence or a business
- Whether you lost access to personal, private or public transportation because of the damage
- Whether it impacted your employment
- Whether the property is in a federally recognized Tribal Nation
- What caused the most damage (wind, water, etc.)
- Insurance information
- Homeowners or Renters insurance
- Flood insurance
- Photographs of damage
- Contact information
